Every patient is unique, our office is committed to providing you with high quality, state-of-the-art cosmetic and family dental care based on your individual needs. Most important thing, we want to make your visit to our office a pleasant experience.
Your First Visit
Your initial appointment will consist of a comprehensive evaluation and any necessary X-rays. During this visit we will discuss your diagnosis and treatment options. Occasionally, treatment can be done the same day as the evaluation. However, a complex medical history or treatment plan can require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your evaluation:
- Any X-rays if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring your insurance card. We can call your insurance provider to determine and discuss your insurance coverage. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics, for pre-med.)
X-Rays
If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

We accept all dental insurance plans and are happy to file insurance forms for you. Please bring your insurance card and booklet with you on your appointment so we can better assist you. We can also call your insurance when you arrive in the office to confirm and ask more detailed information on your insurance coverage.
Your dental plan coverage is determined by you and your employer. We can help you understand your coverage and assist you in obtaining pre-determinations for treatment. You can also discuss details of your plan with your employer’s human resources department.
